The completed application form in duplicate should be submitted along with photo copies of the following:
- PUC pass certificate or its equivalent.
- .Recent conduct certificate from the Principal of the college last attended.
- .Certificates of other educational qualifications if any.
- T.C./Cumulative record, Migration Certificate.
- Six recent passport size photographs and six stamp-sized photographs.
- Vaccination certificate preferably against Hepatitis B, if available.
- Medical fitness Certificate.
- A covering letter in candidate's own handwriting with the list of enclosures.
